Onderzoeksprojecten
Onderzoeksproject 1: Micro-inflammatie en atherogenese in uremie en nierfunctie vervangende therapie.
Het voorgestelde project kadert in het onderzoek naar oorzaken van micro-inflammatie en de hiermee gerelateerde cardiale belasting waaraan de patiënt met chronisch nierfalen wordt blootgesteld. Zowel de retentie van uremische componenten, die normalerwijze door een gezonde nier worden uitgescheiden, als de nierfunctie vervangende therapie kunnen een rol spelen in deze inflammatie.
De impact van gekende retentiestoffen wordt nagegaan in een in vitro screening op leukocyten. Daarenboven zal de rol van specifieke leukocytensubpopulaties en van de pathofysiologische mechanismen ontrafeld worden. Omdat in vitro modellen de cross-talk tussen de verschillende celsystemen echter niet evalueren wordt ook dmv intravitaal microscopie het effect van een acute blootstelling aan geselecteerde uremische retentiestoffen op de interactie van leukocyten met de vaten in het peritoneum van ratten bestudeerd. In deze real-time in vivo set-up wordt de leukocytenrecrutering, -adhesie en -transmigratie bestudeerd.
Dit zal leiden tot een classificatie van de betrokken retentiestoffen op basis van hun toxiciteit. Via een metaboloomstudie wordt gezocht naar nieuwe retentiestoffen en biomerkers. Deze nieuwe merkers kunnen bijdragen tot een vroege diagnose van nierfalen en geven aanleiding tot nieuwe targets voor therapeutische interventies.
Met betrekking tot de dialysetherapie op zich, wordt gezocht naar de ideale methode om de kwaliteit van de dialysaatvloeistoffen te controleren. Accurate detectie van contaminatie zal bijdragen tot een correctie van de micro-inflammatie bij de dialysepatiënten. Bij de transplantpatiënten wordt getracht om via een gepersonaliseerde aanpak de immuunsuppressie te optimaliseren. Hierdoor zal, in geval van overimmuunsuppressie, de kans op infectie en kankerontwikkeling en, in geval van onderimmuunsuppresie, de kans op rejectie gereduceerd worden.
Onderzoeksproject 2: ARCH (PAtient specific image-based computational modeling for impRovement of short- and long-term outCome of vascular access in patients on Hemodialysis therapy).
Onderzoek naar de invloed van het aanbrengen van een arterio-veneuse fistel als vaattoegang voor hemodialyse op de vasculaire en cardiale hemodynamica.
Onderzoeksproject 3: Cochrane-ERA-EDTA project: Collaboration on best practice guidelines.
Systematisch literatuuronderzoek en evidentie quotering inzake depressie and chronisch nierlijden alsook niertransplantatie, met het oog op het opstellen van nieuwe richtlijnen. Dit gebeurt in samenwerking met Cochrane en de European Renal Association - Renal Dialysis and Transplantation Association (ERA-EDTA).
Onderzoeksproject 4: Belgian screening project for the detection of Anderson-Fabry disease in hypertrophic cardiomyopathy.
Onderzoeksproject 5: School-based screening project to detect the prevalence of alpha galactosidase A mutations in boys (6-12 years) and male young adults (12-18 years)
Onderzoeksproject 6: Individually adapted immunosuppression in de novo renal transplantation based on immune function monitoring: a prospective randomised study.
Onderzoeksproject 7: The role of hypomagnesemia posttransplantation.
Onderzoeksproject 8: Multicentrische studie met als titel ‘Complementactivatie tijdens hemodialyse bij atypisch hemolytisch uremisch syndroom als onderliggend nierlijden.
Doel van de studie is nagaan of er tijdens hemodialyse bij patiënten met atypisch hemolytisch uremisch syndroom een significant grotere complement activatie optreedt in vergelijking met patiënten met een ander onderliggend nierlijden.
